Definition:
Acne vulgaris: The common form of acne seen most often in teenagers or young adults, acne vulgaris is the result of overactive oil glands that become plugged, red, and inflamed.

Presentation and Treatment

Acne Vulgaris

comedonal acne, no inflammation

Usual microorganisms: 

Excessive sebum production and gland obstruction. No propionobacterium acnes

Suggested therapy:

Primary :
tretinoin cream (0.025% or 0.05%) qd
tretinoin gel (0.01 or 0.025%) qd
Alternative :
topical adapalene 0.1% cream qd
topical azelaic acid 20% cream qd

mild inflammation, small papules or pustules

Usual microorganisms: 

propionobacterium acnes (proliferation, desquamation)

Suggested therapy:

Primary : 
topical (erythromycin + benzoyl peroxide) bid
Alternative : 
topical (clindamycin 1% + benzoyl peroxide 5%) bid

inflammatory acne: common papules and pustules

Usual microorganisms: propionobacterium acnes (progression of infection)

Suggested therapy:

Primary : 
topical (erythromycin + benzoyl peroxide) bid ± oral Rx
Alternative : Oral drugs
doxycycline 100 mg PO bid or
minocycline 50 mg PO bid or
erythromycin 250-500 mg PO q6h or
tetracycline 250-500 mg PO q6h or
TMP-SMX 1 DS tablet PO bid or
clindamycin 150-450 mg PO q6h or

Acne Rosacea

Usual microorganisms: 

skin mite? (demodex follicurolum)

Suggested therapy:

Primary : 
metronidazole topical 1% cream
Alternative : 
doxycycline 100 mg PO bid
